What happens next
1. We call you
Within one business day of your request. If we miss you, we'll try again and leave a message.
2. A short intake call
About 15 minutes. Our intake team asks about your history, current symptoms, and goes over our policies so nothing is a surprise later.
3. We book your appointment
Together, at a time that works — in the office you chose or by telehealth.
What to bring
- Your insurance card. We don’t collect card images through the website, so please bring your card (or a photo of both sides) to your first visit. See the plans we accept.
- A photo ID.
- A list of any medications you currently take, including doses.
- Your completed registration form, if you filled it out ahead of time.
